Claxton-Hepburn Medical Center Recognizes Case Management Staff

Claxton-Hepburn Medical Center joined the recognition of National Case Management Week October 13-19, 2019, by highlighting its Case Management staff for services they provide for our patients. According to the Case Management Society of America, “Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.” Mike Beldock, director of case management at Claxton-Hepburn stated, “This week is a way to be able to recognize our great case management team that works very hard every day, much of the time behind the scenes, to ensure our patients are safely led through their continuum of care.” He continued, “Please take a little bit of time out with me to thank our team for all their services to Claxton-Hepburn and their patients.” Left to right in the photo is members of our Case Management team as follows; Jennifer Hitsman, LPN, CCP, Jennifer Wright, RN, Jessica Russell, RN, Michael Beldock, BTech, RN, CCM, Shauna Prashaw, RN, CCM, Tammy Drummond RN, and Kareen Bedford, CHAA.
