Our wellness programs are a great way to take control of your health.

I Can Feel Better Program

The "I Can Feel Better" program is a free chronic disease self-management education (CDSME) program designed to help people manage chronic health conditions in practical ways. The group program runs for six-weeks, with classes lasting 2-3 hours. Using evidence-based practices, CDSME participants will gain a strong foundation of self-management skills in an interactive, supportive environment. Participants will learn to manage their chronic conditions utilizing an assortment of tools and techniques to help overcome the symptoms of chronic illness to feel better and do more of the things you like to do.

Diabetes Self-Management Education

The Diabetes Self-Management Education (DSME) program is a free chronic disease self-management program designed for people with diabetes. The program runs for six-weeks, with classes lasting 2-3 hours. The group program uses evidence-based practices to teach participants self-management skills to manage their diabetes and related conditions, on a day-to-day basis. Through good self-management, people with diabetes can improve their quality of life and reduce the risk of developing complications.

Diabetes Prevention Program

The Diabetes Prevention Program (DPP) is a free evidence-based program that provides a supportive environment, where participants work together to reduce their risk of developing diabetes. The participants learn about healthy eating and increasing their physical activity in a classroom setting, that meets weekly for 16 weeks, one-hour sessions, followed by 6 monthly sessions. The DPP showed that people who are at high risk for type 2 diabetes can prevent or delay the disease by losing a modest amount of weight through lifestyle changes.

Diabetes Self-Management Education Individual Counseling

A physician referral is required to meet with a diabetes educator. Depending on the situation, insurance and providers’ preferences, participants may meet with a diabetes educator several times, individually. The diabetes educator will spend time with you developing a plan that helps to overcome the barriers one faces in managing diabetes. The diabetes educator will explain what diabetes is, and hot it affects the body.

Healthy Lifestyle Support Group

Finding it difficult to keep your momentum and motivation when it comes to making healthy choices? This support group encompasses everything about leading a healthy lifestyle including tips on eating well, exercising, and activities available, locally. The group meets monthly, for one hour.
